Role: The RN, Patient Access is responsible for the clinical coordination of any hospice evaluation and or admission to hospice. S/he will address palliative, spiritual, psychosocial, volunteer and emergent care needs of the patient and family in all care settings: home, ALF, Nursing Home, Hospital and or other location. Additionally, the RN will case manage and provide daily care coordination for any hospitalized patient.

Qualifications:

Current Florida license as RN, or current licensure from another state plus a temporary Florida permit.

Valid driver's license and automobile insurance in the State of Florida.

Reliable transportation to meet schedule.

Minimum of three (3) years nursing experience, with at least one (1) year acute care, preferably ER or critical care nursing.

Excellent critical decision making and problem solving skills.

Ability to effectively communicate.

Flexible schedule to meet the needs of hospice patients.

Ability to physically access any care setting, i.e. navigate stairs and narrow spaces, tolerate heat and lack of air conditioning, in any area Company serves.

Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.

Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation.

Job Responsibilities:Clinical

Evaluate every hospice referral on same day as referral, unless otherwise indicated.

Assess patient/family and identify immediate needs including assessing psychosocial, spiritual, and volunteer needs, and make appropriate referrals, as needed.

Initiate the plan of care that accurately reflects the patient's immediate needs.

Evaluate patient for hospice eligibility admission, utilizing approved criteria and guidelines.

Ensures point of service documentation is completed and e-signed within 2 hours of visit end time.

Demonstrate clinical skills regarding physical assessment, symptom management, patient/family relationships and case management.

Consult, refer, and collaborate with other professionals to promote quality and continuity of patient/family care.

Follows orders from physicians and ARNPs to manage hospice patients expeditiously.

Educate patient, family, caregivers and other health professionals about disease process, decline, palliative interventions, care giving, dying process and safety practices.

Provide information regarding available community resources and other community resources when hospice care is not appropriate.

Provide timely and consistent feedback to referral sources, physicians, and patient/families regarding care plan.

Educate case managers, social workers, ALFs, nursing homes and other referral sources to make timely referrals to hospice.

Accept responsibility and accountability for nursing actions: demonstrate an understanding of the legal consequences of nursing actions; request supervision when needed; perform within the guidelines established by the Florida Nurse Practice Act.

Provide home/facility visits in all clinical settings, if no admission visits are scheduled during the regular work day.

Administrative

Document at Point of Service the assessment, care plan, and interventions with accuracy, timeliness, legibility, professional terminology and awareness of legal and regulatory implications.

Document patient condition and decline to support hospice and level of care appropriateness.

Present concise and pertinent oral and written reports to IDG; respect and encourage input from all disciplines.

Provide hospital with a copy of the hospice plan of care when patient will remain in the hospital after date of hospice admission.

Communicate accurately and completely to physicians, staff members, patients, families, and supervisors; utilize positive approaches when working with others.

Exemplify behaviors supportive of company mission statement.

Participate in orienting new Patient Access staff, students, volunteers and board members.

Participate on committees and projects, as assigned.

Act as a resource to peers, students, orientees and other staff members.

Participate in activities and professional development that enhance hospice nursing skills.

Participate in education process of the Company, both to the medical and lay communities. 

Correct any documentation deficiencies on the same day as requested to avoid delays.

Turn in all paperwork within 24 hours of patient visit.

Marketing

Cultivate, develop and enhance care relationships with assigned care partners.

Ensure timely attention to customer concerns and complaints.

Provide outstanding customer service to the company's care partners, both internal and external.

Collaborate with Professional Relations to increase access to hospice care.

Assist in collecting feedback from referral sources.

Maintain membership in related health care professional organizations for the purpose of networking.

Participate in assigned hospital/facility committee(s) as requested by and approved by the Manager, Patient Access.

Education

Participate in education plan for assigned hospital(s)/facility(s) as established by and in conjunction with Professional Relations.

Identify opportunities for hospice in-services in the assigned hospital(s)/facility(s) and coordinate delivery.

Provide formal and informal education opportunities in the hospital/facility settings with staff, physicians, and social workers/discharge planners.

Communicate health care issues to clinical leadership to enhance service delivery.

Customer Service

Develop positive customer relationships in assigned facility(s) by effectively educating hospice care and conducting presentations both 1 on 1 and formal group in-services, which support the Hospice mission.

Provide information about hospice services to all requesting it.

Work cooperatively with all departments to meet the needs of the community and patient/family.

Maintain regular contact with hospital discharge planners and/or social workers and other key hospital staff.

Maintain professional appearance at all times.

Financial

Demonstrate knowledge of hospice reimbursement to bill appropriately and complete necessary paperwork.

Employ palliative and home care management modalities in care of the patient prior to hospitalization or to expedite hospital discharge planning.

Demonstrate knowledge of the different levels of hospice care and work with clinical staff for appropriate utilization of the hospice houses and continuous care as part of the hospice continuum of care.

Utilize supplies, equipment, medications, laboratory and patient therapies within the Company policies.

Communication

Ensure timely attention to customer concerns and complaints. Provide outstanding customer care to patients, families as well as each other.

Communicate accurately and completely to physicians, ARNPs, staff members, patients, families and other supervisors/managers using a positive approach.

Demonstrate respect and understanding for all clinical disciplines required for quality, safe patient care and mentor staff in this process.

Professional Development

Pursue own professional growth through attendance at required in-services and other educational offerings.

Establish professional goals for self and continually evaluate self in the attainment of these goals.

Other

Duties as assigned by the Director or Manager, Patient Access.

Additional RN, Preceptor Responsibilities:Leadership

Serve as a back-up to Patient Access, Clinical Team Leader. 

Education

Plan, coordinate, and carry out educational opportunities for orientee(s).

Explain the role and performance expectation for the RN, Patient Access in the hospice setting.

Assist the orientee to identify learning needs.

Plan for and provide relevant learning experiences to orientee(s) involving the Clinical Team Leader, Senior Nurse Preceptor and other staff, necessary.

Orient new nurses to their roles within the department and IDG.

Evaluate competence of orientee utilizing orientation competency checklist.

Provide prompt and consistent feedback, identifying areas in need of further development to the new nurse and the Clinical Team Leader.

Coordinate activities with other nurses to maintain consistency in practice, establish relationships, and broaden the scope of the orientation.

Provide input to Clinical leadership and educators regarding patient care experiences.

Participate in meetings as assigned.
