- Bachelors degree or above in Social Work (CSWE accredited).
- One year experience in health care/hospital setting required.
- Familiarity with and working knowledge of the concepts and practices of death and dying with children and adults.
- Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload.
- Able to remain calm, professional, trouble shoot and intervene with patient/family crisis situations.
- Valid driver's license and automobile insurance per company policy and procedure.
- Ability to travel to off-site locations.
- Ability to work unsupervised exercising a high degree of discretion & independent judgment.
- Ability to physically access any home, hospital, nursing home, and ALF i.e. navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc.
- Ability to work in a team environment and intervene appropriately.
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system required
- Demonstrate joint responsibility with RN Care Coordinator for pediatric patients, patient transfers (i.e., home-inpatient-home), and coordinating with inpatient staff to assure continuity of care.
- Coordinates/Collaborates Plan of Care activities with Social Services Coordinator and team Clinical Manager.
- Attends staff meetings; in-service training; continuing education and workshops.
- Responsible for Volunteer and other training (Social Services/Bereavement) as assigned.
- Participates in Quality Assessment and Performance Improvement activities as directed.
- Participates in the IDG and provides services to patients and families according to the Plan of Care.
- Participates in Hospice committees as assigned.
- Documents findings on appropriate social services forms in appropriate time frame based on referral from RN Care Coordinator and/or IDG; Provides supportive guidance to patients and families.
- Initiates appropriate case management activities based upon established policy; Works with Resource Associate to complete tasks.
- Experience in children, family, and group modalities.
- Empowers patient/families to negotiate with resource providers and make decisions on their own behalf.
- Completes social services documentation as needed.
- Participates in on-call assignments.
- Maintains documentation in a manner that reflects timeliness, completeness, accuracy, legibility, clarity of goals and interventions.
- Evaluates bereavement needs, including making condolence calls, evaluating risk and coordinating the Plan of Care with IDG and bereavement staff.
- Works with Resource Associates to coordinate concrete patient care needs, to direct patient visits and attend interdisciplinary group (IDG) meetings are required.
- Attends and participates in IDG meetings; assists in developing and updating the Plan of Care; facilitates understanding of social services and bereavement aspects of care, and provides team with goals and interventions for patient/family care.
- Consults and collaborates with other professionals to promote quality care and continuity of patient/family care.
- Demonstrates respect for fellow professionals and IDG leadership.
- Demonstrates critical thinking in problem solving patient/family issues.
- Enhances professional expertise and hospice knowledge.
- Establishes professional goals and evaluates attainment of goals.
- Participates in appropriate professional or specialty organizations.
- Performs other duties as assigned.
- Conduct represents the NASW Code of Ethics.
- Works within HPC Scope of Service.
