Role:

The Clinical Team Leader (CTL) is responsible for the daily clinical coordination of the Interdisciplinary Group (IDG) in delivering palliative, supportive and emergent care to the patient/family unit as indicated in the plan of care.

 

Qualifications:

  • Current Florida license as RN, or current licensure from another state plus a temporary Florida permit
  • Minimum of three (3) years nursing experience, with at least one (1) year acute and medical surgery nursing
  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system required
  • Valid driver's license and automobile insurance per Company policy
  • Reliable transportation to meet visit schedule
  • Excellent critical decision making and problem solving skills
  • Ability to use equipment with visual and auditory mechanisms
  • Ability to effectively communicate in English
  • Flexible schedule to meet the needs of hospice patients
  • Ability to physically access any home (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning) in any area the Company serves
  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families

  • Job Responsibilities:
  • Clinical
  • Act as primary clinical resource for clinical staff and IDG.
  • Obtain report and monitor ongoing assessments of physical, psychological, social and spiritual needs of the patient, family and patient caregiver and initiate appropriate intervention or referral in collaboration with IDG.
  • Recommend alternate approaches to patient care as indicated.
  • Provide education to IDG members regarding symptom control, case management, Company, community and financial resources guided by Policy and Procedure.
  • Enhance the clinical practice of IDG staff by performing as a role model, mentor, instructor and resource person.
  • Provide coverage/assistance to staff as needed and as patient acuity dictates to ensure continuity of care to all patients.
  • Coordinate patient care, including assessing psychosocial, spiritual care, and volunteer needs, and make appropriate referrals, as needed.
  • Coordinate the plan of care, ensuring that an individualized plan of care is developed that accurately reflects the patient's evolving needs.
  • Demonstrate clinical skills regarding: case management, application of company policy and procedure,         physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships.
  • Take and follow verbal orders from physicians, ARNPs and physician's assistants.
  • Educate patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process and safety practices.
  • Attend patient deaths; prepare body for transport; oversee disposal of medications.
  • Provide appropriate bereavement interventions to the family and caregiver in conjunction with psychosocial           staff.
  • Accept responsibility and is accountable for nursing actions: demonstrate an understanding of the legal consequences of nursing actions; request supervision, when needed; perform within the guidelines established by the Florida Nurse Practice Act.Operations       
  • Supervise patient care given by other nursing team members (i.e., LPNs, hospice aides, and contract nurses).
  • Assist with determining daily utilization of staff.
  • Participate in departmental planning.
  • Evaluate quality of care delivered by staff; work collaboratively with manager to identity staff needs; assist staff in meeting patient/family needs.
  • Collaborate with other departments (i.e., Medical Services, Pharmacy, Infusion Program, and Continuous Care) to ensure patient care needs are addressed.Communication  
  • Communicate in a clear and accurate manner. Provide constructive feedback and resolve conflict directly. Assist others to improve communication skills.

 

  • Incorporate joint decision-making and unified goal achievement into IDG practice.
  • Demonstrate respect and understanding for all clinical disciplines required for quality patient care and mentor staff in this process.
  • Ensure timely attention to customer concerns and complaints. Provide outstanding customer care to patient and families.
  • Document assessment, plan, intervention and evaluation on appropriate Company forms with accuracy, timeliness, legibility, professional terminology and awareness of legal and regulatory implications.
  • Document patient condition and decline to support hospice and level of care appropriateness.
  • Communicate accurately and completely to physicians, staff members, patients, families, and supervisors; utilize positive approaches when working with others.
  • Exemplify behaviors supportive of the mission statement.
  • Submit paperwork in a timely manner.
  • Participate on committees and projects, as assigned.

  • Assist in reviewing patient care documentation and providing on-site clinical supervisory visits to identify strengths and areas of development for the clinical staff.
  • Supervise clinical staff to assess individual competency.
  • Promote professional development of all members of the IDG.
  • Recognize, reinforce, and enhance the role of all disciplines in the IDG.
  • Identify learning needs of clinical staff based on assessments and observations and assist manager to develop educational plans to meet the needs.
