Role:

The Pediatric Clinical Team Lead (CTL) is responsible for the daily clinical coordination of the Pediatric and Partners in Care (PIC) team in delivering palliative, supportive and emergent care to the patient/family unit, as indicated in the plan of care.

 

Qualifications:

  • Current Florida license as RN, or current licensure from another state plus a temporary Florida permit
  • Minimum of three (3) years' nursing experience, with, at least, one (1) year in acute pediatric nursing
  • Valid driver's license and automobile insurance per Company policy
  • Reliable transportation to meet visit schedule
  • Excellent critical decision making and problem solving skills
  • Demonstrated skills regarding: case management, application of policies and procedures, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships.
  • Ability to effectively communicate in English
  • Flexible schedule to meet the needs of hospice patients
  • Ability to physically access any home i.e. navigate stairs and narrow spaces, tolerate heat and lack of air conditioning, in any area Company serves
  • Ability to manage the emotional stress of working with and caring for terminally ill pediatric patients and their families
  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation
  • Ability to manage time effectively to ensure that both clinical and operational/leadership aspects of role are completed; role consists of 60% clinical and 40% operations

  • Act as RN-Care Coordinator and Preceptor for department personnel and support department. Act as clinical resource for pediatric and PIC patients on the team wherever the patients are located.
  • Act as clinical resource for pediatric and PIC Interdisciplinary Group (IDG).
  • Recommend alternate approaches to patient care.
  • Comply with AHCA PIC Program regulations and requirements.
  • Provide education to department members regarding symptom control, case management, Company, community and financial resources guided by Policies and Procedures.
  • Enhance the clinical practice of department staff by performing as a role model, mentor, preceptor and resource person.
  • Recommend palliative and home care management modalities in care of the patient prior to hospitalization.
  • Facilitate/participate in pediatric and PIC complex patient care conferences, as necessary, to facilitate meeting expected outcomes established in the plan of care.
  • Assist team and staff members to accurately determine patients' clinical needs, along with the appropriate level of care.
  • Provide coverage/assistance to staff, as needed, and as patient acuity dictates to ensure continuity of care to all patients. Communicate coverage/assistance to the Clinical Manager, as needed.
  • Coordinate patient care, including assessing psychosocial, spiritual care, and volunteer needs, and make appropriate referrals, as needed.
  • Take verbal orders from physicians, ARNPs and physician's assistants.
  • Facilitate seamless coordination of patients' care between all parties, internal and external.
  • Educate patient, family, caregivers and department employees about disease process and decline, palliative interventions, care giving, dying process, and safety practices.
  • Accept responsibility and is accountable for nursing actions: demonstrate an understanding of the legal consequences of nursing actions; request supervision, when needed; perform within the guidelines established by the Florida Nurse Practice Act.Operations -40% of FTE

  • Assist with determining daily utilization of staff.
  • Evaluate quality of care delivered by staff; work collaboratively with Manager to identity staff needs; assist staff in meeting patient/family needs.
  • Supervise care given by staff members as assigned, i.e., RNs, LPNs, Hospice Aides, SSSs, Chaplains.
  • Collaborate with other teams and departments, i.e., Medical Services, Pharmacy, Infusion Program, DME, IDG, and clinical teams, to ensure patient care needs are addressed and met.
  • Participate on committees and projects, as assigned.
  • Monitor team's performance, e.g., staff utilization and patient pay classes, status changes, and patient care expenses related to hospitalizations, medications, supplies.
  • Assist in providing regular reports on productivity and other performance measures.

 

  • Assist in determining agenda and conducting staff meetings with direction from the Manager. Ensure effective and timely staff meetings.
  • Assist CM to inform staff members about clinical and Company business matters including new/revised policies and procedures.
  • Implement assigned component of the Organizational disaster plan.
  • Ensure all required processes are completed in a timely manner to support regulatory compliance, e.g., recertification updates, care plans, physician orders.Communication

  • Carry out other responsibilities including conflict resolution, assist with coaching/counseling, and performance evaluations in conjunction with Clinical Manager.
  • Perform on-site performance assessments of staff, as needed, and as directed by the CM.
  • Assist in coordinating completion of employee training requirements (due/past due training).Quality Assessment Performance Improvement (QAPI)
  • Identify QAPI opportunities in patient care and other areas of supervision, including risk trends/patterns.
  • Assist in ensuring that the processes for identifying and mitigating sentinel events are clearly communicated to and utilized by staff members
  • Conduct and review PIC patient surveys, as required.Staff Development
  • Assist in reviewing patient care documentation and providing on-site clinical visits to identify strengths and areas of development for the clinical staff.
  • Promote professional development of all members of the department.
  • Identify learning needs of staff based on assessments and observations and assist manager to develop educational plans to meet the needs.
  • Provide written/verbal input to the evaluation of department staff.Customer Service and Marketing
  • Provide outstanding customer service to the Company's customers, both internal and external.
  • Recommend to Manager possible suggestions to improve customer satisfaction.
  • Assist in collecting feedback from referral sources.
  • Assist in carrying out admissions and marketing plans in service area(s).Other
