Role: The Clinical Team Leader is responsible for the daily clinical coordination of the Interdisciplinary Group (IDG) in delivering palliative, supportive and emergent care to the patient/family unit as indicated in the plan of care.

 

Qualifications:

  • Current Florida license as RN, or current licensure from another state plus a temporary Florida permit.
  • Valid driver's license and automobile insurance in the State of Florida.
  • Reliable transportation to meet visit schedule.
  • Minimum of three (3) years nursing experience, with at least one (1) year acute and medical surgery nursing.
  • Excellent critical decision making and problem solving skills.
  • Ability to effectively communicate in English.
  • Flexible schedule to meet the needs of hospice patients.
  • Ability to physically access any home i.e. navigate stairs and narrow spaces, tolerate heat and lack of air conditioning, in any area Company serves.
  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation.
  • Job Responsibilities:

    Clinical

    • Act as primary clinical resource for department personnel and support department preceptors.
    • Recommend alternate approaches to patient care as indicated.
    • Provide education to department members regarding symptom control, case management, Company, community and financial resources guided by Policy and Procedure.
    • Enhance the clinical practice of department staff by performing as a role model, mentor, instructor and resource person.
    • Provide coverage/assistance to manager as needed and as patient acuity dictates to ensure continuity of care to all patients.
    • Demonstrate skills regarding: case management, application of company policy and procedure,         physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships.
    • Educate patient, family, caregivers and department employees about disease process and decline, palliative interventions, care giving, dying process and safety practices.
    • Accept responsibility and is accountable for nursing actions: demonstrate an understanding of the legal consequences of nursing actions; request supervision when needed; perform within the guidelines established by the Florida Nurse Practice Act.

     

    Operations       

    • Assist with determining daily utilization of staff.
      • Participate in departmental planning.
      • Evaluate quality of care delivered by staff; work collaboratively with manager to identity staff needs; assist staff in meeting patient/family needs.
      • Collaborate with other departments, i.e., Medical Services, Pharmacy, Infusion Program, and clinical teams, to ensure patient care needs are addressed.
      • Participate on committees and projects, as assigned.

     

    Communication

    • Communicate in a clear and accurate manner.  Provide constructive feedback and resolve conflict directly.  Assist others to improve communication skills.
    • Incorporate joint decision-making and unified goal achievement into the department.
    • Demonstrate respect and understanding for all clinical disciplines required for quality patient care and mentor staff in this process.
    • Ensure timely attention to customer concerns and complaints. Provide outstanding customer care to patient and families.
    • Communicate accurately and completely to physicians, staff members, patients, families, and supervisors; utilize positive approaches when working with others.
    • Supervise care given staff members as assigned; i.e., RNs, LPNs, hospice aides, SSS, Chaplains.
    • Exemplify behaviors supportive mission statement.
    • Submit paperwork in a timely manner.
    • Act as a resource to peers, students, orientees and other staff members.

     

    Financial

    • Demonstrate knowledge of payment structures and paperwork necessary to change financial classes.
      • Utilize staffing, supplies, equipment, medications, laboratory and patient therapies within the Company philosophy.
      • Maintenance and approve Solutions, CellTrak and SanTrax payroll systems

     

    Staff Development

    • Assist in reviewing patient care documentation and providing on-site clinical supervisory visits to identify strengths and areas of development for the clinical staff.
    • Promote professional development of all members of the department.
    • Identify learning needs of staff based on assessments and observations and assist manager to develop educational plans to meet the needs.
    • Provide written/verbal input to the evaluation of department staff 

     

    Professional Development

    • Pursue own professional growth through attendance at required in-services and other educational offerings.
    • Establish professional goals for self and continually evaluate self in the attainment of these goals.Participate in activities and professional development that enhance skills.Participate in the broad education process of the Company, both to the medical and lay communities.
